|Ancient Core||Obtained from Golem as loot||If the player's sentry number is more than or equal to 2, sentries that deal "contact" damage have a chance to release blue scarabs when hitting an enemy. If it is more or equal than 4, their defense is increased by 3 depending on the number of active sentries, and their life regen is increased by one point less than the active sentry number. If it is more or equal than 6, it will summon a pair of hands and a head, all resembling the Golem's hands and head will appear to the sides and above the player, shooting yellow lasers and punching nearby enemies, for approximately 60~80 damage that doesn't benefit from any bonuses.|
|Angelic Amulet||Bought from the Fallen Angel||If the player takes damage greater than 100, it will nullify that damage and activate a cooldown, giving the player the Angelic protection colldown debuff to signal that cooldown. Increases life by 10, and increases life regen by 1 if the player has less than 40% health.|
|Soul of Integrity||Obtained from the Politician Slime as loot||Gives the player the "control of gravity", allowing them to go up and down in a Gravitation Potion manner. Increases defense and life regen by 1, and damage reduction by 5%. Gives immunity to the Distorted and Jumpscared debuffs.|
|Bottle of Boiling Blood||Obtained from Okiku as loot||When equipped, a heart appears above the player, shooting blood (White puffs of smoke if Gore and Blood is disabled) in random directions in short periods of time. When the blood hits an enemy, it gives them the On Fire! debuff. If an enemy touches the heart, it gives them the Penetrated debuff, making them also shoot blood in random directions, similar to the heart.|
|Caesar's Crown||Obtained from Hoplites and Medusas after the Eater of Worlds or the Brain of Cthulhu is defeated||The player takes 5% less damage from their front, but will take 5% more damage from their back. Depending on the minion amount that the player has, it increases life, mana and defense. Grants 5% extra summon damage. The player will take less damage from pirates and "undead" enemies, such as Skeletons and Zombies.|

|Chef's Skull||Obtained from the Undead Chef in the Dungeon||Increases life and mana by 20. Increases the healing provided by all the Healing Potions by 10%, and they give the Well Fed buff. Mana potions restore 10% more mana, and buff potions have a 20% chance not to be consumed.|
|Concentrated Perfect Gel||Obtained by the Knights of Revenge as loot||Slowly increases the player's stats over time. At max level, it grants immunity to the Chilled debuff. If the player unequips it or dies, all stats will be reset.|
|Mystical Stones||Obtained from the Ancient Guardian as loot||Depending on the damage type of the weapon that the player has in hands, it offers different abilities.
If the player is using a magic weapon, every 5 shots the player will shoot a shadowflame skull, which the damage of said skull scales with weapon damage; If the player is holding a summoner weapon, a small washing machine is summoned, acting in a manner similar to the Baby Slime, quickly dealing 1 damage to enemies that it comes in contact with.
If the player is using a Ranged weapon, a blade appears approximatedly 2~3 blocks from the player, which the player can control with their mouse. The blade acts as if the player was an axis, and it rotates around the player depending on the mouse position. The blade does 25 damage while it is in contact with an enemy.
If the player is using a melee weapon, any kind of Hearts heal 4 more points of health.
|If the player's maximum mana is higher than the mana needed for a magic weapon, the player won't consume mana. When the player hits an enemy with a magic weapon, the enemy is inflicted with Shadowflames and they will spawn 1 projectile that comes back to the enemy and deals 27-29 damage. The player will lose 10% of their maximum health, and 25% of their life regen.|

|Increases Armor Penetration by 50 and Defense by 5.|
|Soul of Kindness||Dropped from Skeletron as loot||When equipped, it generates a green shield around the player, that if any enemy comes in contact with, it deals 10-14 damage. When the player is struck, they gain the Shield of Kindness buff, of which increases defense by 5.
Increases Life Regen, Defense and Endurance by 2, 3 and 5%, respectively.

|When equipped, grants 5 Defense, and immunity to the On Fire! Debuff. If an enemy does contact damage to the player, it gives the enemy the On Fire! buff, and it has a 20% chance to shoot 3 fireballs in random directions. If the player uses it like a consumable item, the player will be able to craft Mythril Anvil/Orichalcum Anvil anywhere, without the need of a crafting station. Closing and opening the game resets this effect.|
|Air Force Medal of Honor||Obtained from the Royal Harpy Air Force as loot||When equipped, it gives immunity to the Weak and Confused debuffs, 5% bonus to the Magic and Melee damage types, and it summons a Harpy to follow the player that periodically shoots feathers at enemies, dealing 7-8 damage.|
|Idol||Obtained from the Wandering Samurai after the Golem is defeated||Adds 2 defense per active minions that the player has. If the player dies, their minions will be replaced by a minion with a Dungeon Spirit sprite that shoots souls at enemies, dealing the same damage that the "previous" minion did.|
|Rune of Impulse||Obtained from the Lunatic Cultist as loot||Makes the player's attacks ignore enemy Immunity Frames, in exchange of all the player's life regeneration.|
|Jellyfish Tentacle||Obtained from any Jellyfish as loot||When equipped, the player generates an area of eletricity, that if any enemies get inside, the area deals 16 damage to those enemies, and they are inflicted with the Charged debuff. If a Charged enemy is nearby any other enemies, that Charged enemy transmits energy to the other enemy, dealing 17-22 damage to the struck enemy.|
|Judas' Shadow||Obtained from Isaac as loot||If the player dies with this accessory equipped, they will be inflicted with the Dark Judas debuff, reviving them and setting their life to 40. When inflicted with Dark Judas, their damage is increased by 4%. Their sprite will also be completely black.|
|Lucky Cat||Bought from the Wandering Samurai after the Knights of Revenge are defeated||Every enemy that the player kills has a 10% chance to drop double the amount that they previously dropped.
When the player equips this, a giant coin appears above their head, shooting coins at nearby enemies that deal 37-48 damage in short intervals of time.
|When the player takes a hit, many stars fall from the sky, dealing 68-99 damage to any enemies they strike.|
|When the player takes a hit, many boulders fall from the sky, dealing 100-130 damage to any enemies they strike. Players take less damage than normal boulders.|
|Mana-Charged Crystal Ball||5
|When equipped, it offers 15 mana regen, 20 mana and life points, a permanent Clairvoyance buff and 7% increased magic damage. If the player is in the jungle, these stats are increased to 20 mana regen, 30 mana and life points, Clairvoyance and 12% increased magic damage.|
|Martian Sentry Charger||Obtained from Martian Engineers as loot||Offers 1 extra sentry minion slot and 5% damage reduction. It grants sentries a chance to shoot "concentrated energy" shaped like boomerangs that deal the same damage that the sentries do.|
|MEAT!||Obtained from Isaac as loot||Increases life by 20 and all types of damage by #%.|
| Monster Heart||Obtained from killing any enemy with the Soul of Fear equipped.||When equipped, the player's minions gain a 5% chance to critically hit enemies.|
|Essence of OwO||
|Offers 5% Damage Reduction, 10 life points, and 1 life regen.
Town NPCs have more defense when equipped.
If the player has less than 20% health, the player will receive a bonus to melee, ranged and throwing damage types, movement speed, and they will receive a Night Owl-like vision enhance.
|Pendant of the Crusader||Obtained from the Holy Knight when exchanging the Paladin's Shield||If the player takes damage that is more than 5% of their health, 2% of that damage is nullified. When the total of the absorbed damage is 18%, it will stop nullifying damage. If the player doesn't take damage for 5 seconds, the total damage is reset. If the player takes damage that is less than 5% of their health, the total damage is also reset.|
|Increases critical hit chance for all weapons (Except for summons) by 8% and defense by 2. Improves the player's vision.|
|Rainbow Heart Necklace||30
|When hitting an enemy with a melee attack, that enemy is inflicted with Shadowflame, On Fire!, Cursed Inferno, and Frostburn. Increases life by 40. |Bottled G.P. Bomb||Obtained from the Royal Harpy Force as loot||When equipped, rockets will fall in a radius around the player, dealing 30-35 damage. If an enemy is within an area of a rocket, that rocket will quickly ricochet to that enemy.|
|SCP-714||Obtained from Hayco the Hunter's Duck Hunt mini-game.||When equipped, the player will lose 8% of their Movement Speed, but they will be immune to the Silenced, Confused, Jumpscared, and Weak debuffs. The player will take 10% less damage from projectiles that have Bio, Hazard, Plague, and Chemical in their names.|
|Slimy Tactics||Bought from the Wandering Samurai after the Slime's Last Hope is defeated.||Offers a 5% boost to melee attack speed and throwing projectile velocity. If the player takes damage, they will gain a 5% chance to dodge the next attack. If they do not dodge that attack, it is increased by 5%. This effect stacks until the player takes damage. If the player dodges an attack, the counter is reset.|
|Snowflake Necklace||Dropped from the Pirate Town NPC||Offers 3% damage reduction, 2 defense, and 5% movement speed. Healing potions heal for 20 more health, and Mana potions give 50 more mana.|
|When the player uses a sentry that deals "contact damage" to enemies that come in contact with it, it inflicts the enemy with Electric Charge, a debuff that deals 5 damage in short periods of time. If the sentry stays for 10 seconds, the Electric Charge debuff makes enemies shoot projectiles towards nearby enemies.|
|Slimy TENG||Obtained from the Slime's Last Hope as Loot||When the player is underwater, any attacks will inflict enemies with Charged. If a Charged enemy is nearby any other enemies, that Charged enemy transmits energy to the other enemy, dealing 17-22 damage to the struck enemy. If an enemy is wet, whenever that enemy is inflicted with Charged, they do not need an nearby enemy to transmit eletricity, striking themselves with eletricity. It also grants the player a chance of receiving mana when hitting an enemy with any weapons, effectively reducing the mana cost of magic weapons.|
|Vigor Booster||Obtained from the Slayer of Evil as loot||Increases buff durations by 50%
Getting hit grants a random vanilla buff for 5 seconds
|Depending on the movement key that the player is holding, it increases different stats.
If the player holds UP, their movement speed increases by 20%.
If the player holds DOWN, Their life regen is increased by 6.
If the player holds LEFT, their defense increases by 20.
If the player holds RIGHT, Thier damage reduction increases by 10%
Depending on the game's music volume, their stats can increase or decrease. If it is above 40%, their stats increase, and if it is below 20%, their stats decrease. With a minimum of -50% on all damage types, and with a maximum of: 8% on all damage types.
Three musical notes rotate around the player, with the closest note being the slowest and the farthest one being the fastest. The closest note is able to produce healing notes and Magical Harp's damaging notes. All of them deal 90-120 damage while they are in contact with an enemy.
